La <strong data-start=\"3392\" data-end=\"3415\">syndrome d\u2019Asherman<\/strong>, marqu\u00e9e par la formation d\u2019adh\u00e9rences intra-ut\u00e9rines apr\u00e8s une chirurgie ou une infection, en est un exemple typique. M\u00eame apr\u00e8s des traitements hormonaux ou chirurgicaux, de nombreuses femmes ne parviennent pas \u00e0 obtenir une am\u00e9lioration suffisante.<\/p>\n<p data-start=\"3671\" data-end=\"3973\">Une r\u00e9cente <strong data-start=\"3683\" data-end=\"3744\">\u00e9tude clinique randomis\u00e9e, contr\u00f4l\u00e9e et en double aveugle<\/strong> (Hou et al., 2024) a explor\u00e9 une nouvelle approche : l\u2019utilisation de <strong data-start=\"3815\" data-end=\"3892\">cellules souches m\u00e9senchymateuses d\u00e9riv\u00e9es du cordon ombilical (hUC-MSCs)<\/strong> int\u00e9gr\u00e9es dans un <strong data-start=\"3911\" data-end=\"3936\">scaffold de collag\u00e8ne<\/strong>, implant\u00e9 directement dans l\u2019ut\u00e9rus.<\/p>\n<h2 data-start=\"3980\" data-end=\"4031\">La strat\u00e9gie r\u00e9g\u00e9n\u00e9rative : la \u201cfarming therapy\u201d<\/h2>\n<p data-start=\"4033\" data-end=\"4065\">La <em data-start=\"4036\" data-end=\"4053\">farming therapy<\/em> associe :<\/p>\n<ol data-start=\"4066\" data-end=\"4233\">\n<li data-start=\"4066\" data-end=\"4148\">\n<p data-start=\"4069\" data-end=\"4148\"><strong data-start=\"4069\" data-end=\"4102\">L\u2019adh\u00e9siolyse hyst\u00e9roscopique<\/strong> pour enlever les adh\u00e9rences intra-ut\u00e9rines.<\/p>\n<\/li>\n<li data-start=\"4149\" data-end=\"4233\">\n<p data-start=\"4152\" data-end=\"4233\"><strong data-start=\"4152\" data-end=\"4210\">L\u2019application intra-ut\u00e9rine d\u2019un scaffold de collag\u00e8ne<\/strong> enrichi en hUC-MSCs.<\/p>\n<\/li>\n<\/ol>\n<p data-start=\"4235\" data-end=\"4478\">Ce support biomim\u00e9tique permet aux cellules de s\u2019implanter et de lib\u00e9rer localement des facteurs bioactifs, afin de stimuler la r\u00e9g\u00e9n\u00e9ration de l\u2019endom\u00e8tre, d\u2019augmenter son \u00e9paisseur et d\u2019am\u00e9liorer la r\u00e9ceptivit\u00e9 \u00e0 l\u2019implantation embryonnaire.<\/p>\n<h2 data-start=\"4485\" data-end=\"4508\">R\u00e9sultats de l\u2019\u00e9tude<\/h2>\n<p data-start=\"4510\" data-end=\"4593\">L\u2019\u00e9tude a port\u00e9 sur 24 femmes r\u00e9parties en deux groupes (traitement vs contr\u00f4le).<\/p>\n<ul data-start=\"4595\" data-end=\"4977\">\n<li data-start=\"4595\" data-end=\"4709\">\n<p data-start=\"4597\" data-end=\"4709\"><strong data-start=\"4597\" data-end=\"4638\">Taux cumul\u00e9 de grossesses cliniques :<\/strong> 45,5 % dans le groupe hUC-MSCs contre 7,7 % dans le groupe contr\u00f4le.<\/p>\n<\/li>\n<li data-start=\"4710\" data-end=\"4768\">\n<p data-start=\"4712\" data-end=\"4768\"><strong data-start=\"4712\" data-end=\"4745\">Taux de naissances vivantes :<\/strong> 27,3 % contre 7,7 %.<\/p>\n<\/li>\n<li data-start=\"4769\" data-end=\"4872\">\n<p data-start=\"4771\" data-end=\"4872\"><strong data-start=\"4771\" data-end=\"4801\">\u00c9paisseur de l\u2019endom\u00e8tre :<\/strong> augmentation significative de 5,3 mm \u00e0 6,5 mm dans le groupe trait\u00e9.<\/p>\n<\/li>\n<li data-start=\"4873\" data-end=\"4977\">\n<p data-start=\"4875\" data-end=\"4977\"><strong data-start=\"4875\" data-end=\"4889\">S\u00e9curit\u00e9 :<\/strong> aucun effet ind\u00e9sirable grave ; seulement deux cas d\u2019urticaire, r\u00e9solus spontan\u00e9ment.<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"4979\" data-end=\"5131\">M\u00eame si la significativit\u00e9 statistique n\u2019a pas \u00e9t\u00e9 atteinte (taille d\u2019\u00e9chantillon r\u00e9duite), la tendance positive r\u00e9v\u00e8le un potentiel clinique important.<\/p>\n<h2 data-start=\"5138\" data-end=\"5180\">Comment agissent les cellules souches ?<\/h2>\n<p data-start=\"5182\" data-end=\"5482\">Les analyses du liquide ut\u00e9rin montrent que les cellules souches semblent <strong data-start=\"5256\" data-end=\"5285\">moduler l\u2019immunit\u00e9 locale<\/strong> via les voies cytokines\u2013r\u00e9cepteurs. Cela pourrait r\u00e9duire l\u2019inflammation, stimuler la r\u00e9g\u00e9n\u00e9ration cellulaire et vasculaire, et cr\u00e9er un environnement plus favorable \u00e0 l\u2019implantation embryonnaire.<\/p>\n<h2 data-start=\"5489\" data-end=\"5510\">S\u00e9curit\u00e9 confirm\u00e9e<\/h2>\n<p data-start=\"5512\" data-end=\"5692\">Les examens sanguins, h\u00e9patiques, r\u00e9naux et hormonaux ont confirm\u00e9 la <strong data-start=\"5582\" data-end=\"5615\">bonne tol\u00e9rance \u00e0 court terme<\/strong>. Les effets secondaires (urticaire, naus\u00e9es) \u00e9taient l\u00e9gers et transitoires.<\/p>\n<h2 data-start=\"5699\" data-end=\"5714\">Perspectives<\/h2>\n<p data-start=\"5716\" data-end=\"5907\">Cette \u00e9tude pilote ouvre la voie \u00e0 une nouvelle \u00e8re de <strong data-start=\"5771\" data-end=\"5804\">m\u00e9decine r\u00e9g\u00e9n\u00e9rative ut\u00e9rine<\/strong>, offrant un nouvel espoir aux femmes dont l\u2019endom\u00e8tre reste mince malgr\u00e9 des traitements classiques.<\/p>\n<p data-start=\"5909\" data-end=\"6052\">Des essais plus larges et prolong\u00e9s seront n\u00e9cessaires pour confirmer ces r\u00e9sultats et \u00e9valuer les effets sur la sant\u00e9 maternelle et n\u00e9onatale.<\/p>\n<p data-start=\"6059\" data-end=\"6253\"><strong data-start=\"6059\" data-end=\"6072\">R\u00e9f\u00e9rence<\/strong><br data-start=\"6072\" data-end=\"6075\" \/>Hou Z. et al., <em data-start=\"6090\" data-end=\"6229\">hUC-MSCs loaded collagen scaffold for refractory thin endometrium caused by Asherman syndrome: a double-blind randomized controlled trial<\/em>, 2024.
